With more than 200 colleagues from offices in Ashton under Lyne, Leigh and in our supported accommodation projects across the North West, we support over 7,000 people annually through the contracts we manage.

Role Information:
You will be based at Reubens Court, Leigh. It is a supported housing scheme for single people aged 18 or over with complex needs including mental health.
As a key worker you will have a crucial role supporting people to overcome the barriers they face.
You will work with people to establish their needs and help create a personalised action plan for change, providing advocacy, emotional and practical support and helping them move to permanent or specialised accommodation.
You will be a strong advocate for asset based and trauma informed care and will co-ordinate a package of support, engaging with relevant support, specialist and statutory services throughout.
You will complete regular reviews both in person and over the phone, ensuring that the customer’s action plan is delivered, helping improve their situation and empowering them to move forward with their life.
